NEET SS 2020 Analysis
Exam – Feeder questions were random picks from all feeder subjects and many of these were unexpected out-of-box questions but the best possible way to cover these is by doing PGMEE MCQ books of these subjects. The hematology part was relatively easy with mostly superficial questions.

The exam paper consists of 2 parts
Part A – Has 40 Questions from Feeder subjects which are – Medicine + Pediatrics + Pathology + Biochemistry – MCQs from all four subjects are there irrespective of your feeder qualification.
Part B – Has 60 Questions from Hematology (Hematopathology + Clinical Hematology combined)
What to follow?
1. Feeder Subjects – Follow and solve PG Level MCQ books for these 4 subjects Medicine/Pediatrics/Pathology/Biochemistry. Most of these Qs can be covered by solving these PGMEE MCQ books only. Examples are given below
Medicine – Mudit Khanna/Deepak Marwah
Pediatrics – Meenakshi Bothra(Jaypee)
Pathology – Vandana puri tiwari (CBS)
Biochemistry – Rebbaca James
2. Hematology – You need to cover Clinical Hematology as well as Hematopathology part irrespective of whether you are from path or med/paed.
Books – Hoffbrands Postgraduate Hematology and Or Bathesda Handbook of clinical hematology is best to cover clinical hematology with some hematpathology part hence it is highly recomended + Cover in detail the Hematology part given in Harrison medicine textbook.
Med/Pediatrics residents should additionaly read Hematology part from Robbins Pathology book and solve Hematology part from some pathology PGMEE MCQ book in detail. Pathology residents may also do above to refresh their memories.
MCQ BOOK for HEMATOLOGY: MCQs in Hematology 2nd Edition 2020 by Sanjeev Kumar Sharma & Pawan Kumar Singh can be followed.

Hemolytic uremic syndrome (HUS) is a thrombotic microangiopathy characterized by intravascular hemolysis, thrombocytopenia, and acute kidney failure.
Hemolytic uremic syndrome (HUS) is one of the disease processes that belong to thrombotic microangiopathies (TMAs)
 Typical HUS (ie, STEC-HUS) follows a gastrointestinal infection with STEC
 aHUS is associated primarily with mutations or autoantibodies leading to dysregulated complement activation.
Note = Among the 30% to 50% of patients with aHUS who have no detectable complement defect, some have either impaired diacylglycerol kinase ε (DGKε) activity, cobalamin C deficiency, or plasminogen deficiency.
In aHUS, mutations have been reported in 5 central complement proteins [Factor H, MCP (CD46), Factor I, C3, Factor B]. Most of these mutations lead to impaired regulation of alternative pathway activation. Most commonly involved gene is of Factor H (25-30% of cases)
Historically, aHUS had a poor prognosis with about 50% of all patients progressing to end-stage renal disease (ESRD).
Prior to the introduction of eculizumab, plasma infusion or plasma exchange (PI/PE) was the standard of care by supplying the normal/functional complement inhibitory molecules, but they do not address the underlying pathology.
Eculizumab, a terminal complement inhibitor that binds to C5 with high affinity, blocks the generation of proinflammatory C5a and C5b-9. For me RGCI Delhi and TMC Kolkata are good DNB institutes. With many seats getting added this year the ranks for seat allotment will go beyond rank 54.
